Why might Knowledge Warehouse documents become inaccessible after the upgrade, and how do you prevent it?

Documents not referenced by a structured object can't be activated and become inaccessible afterward.

Before activation, find and reattach them — or archive/delete what you don't need — otherwise you risk losing documents.
