Why does the default test-case execution duration matter beyond just being displayed?

Because it feeds the Scope and Effort Analyzer (SEA) to size test scope — summing case durations and comparing manual vs automated operations.

You can maintain durations per test case and per test-case type (used when the attribute isn't set).
